LVCC has been serving Lake View residents, businesses and visitors since 1952.  As we conclude our 2006-'07 year, we ask you complete this 10-question survey to enable us to understand better how we should communicate with you.  Your feedback is invaluable to us, and we thank you in advance for your help. 
 
By completing the LVCC Satisfaction Survey before June 23, 2007 and being an opted-in member of our email mailing list, you will be automatically entered* into a drawing to win a pair of tickets on the field to see The Police perform at Wrigley Field on Friday, July 6, 2007.   Please forward this email (using the link below) to your friends and neighbors so they can enter the drawing and stay informed about all the happenings in the Lake View area.

For additional entries in the drawing for the two tickets to see the Police, you can purchase tickets to "An Evening at Wrigley" or purchase a brick for the Space Park Recognition Wall.
 
LVCC - The Police Ticket Give Away Terms and Conditions

  1. The LVCC - The Police Ticket Give Away is open to legal residents of the zip codes 60613, 60614, 60640 and 60657 only who are 21 or older as of June 1, 2007.   No purchase is required to participate.   The Give Away is open from June 3, 2007 to June 23, 2007 inclusive.   LVCC Board of Directors for the 2006-2007 term and their immediate families are not eligible to participate.

  2. To receive an entry from the LVCC Satisfaction Survey, you must complete all 10 questions in the LVCC Satisfaction Survey at the above link by no later than midnight CST, June 23, 2007.  Limit one entry per computer.   Limit one entry per email address.  Only completed surveys from individual email addresses who have opted-in to receive communications from LVCC will be considered.

  3. Individuals who purchase tickets online at lakeviewcitizens.org/wrigley.asp to "An Evening at Wrigley" fundraiser will receive 5 entries per ticket purchased before midnight CST, June 23, 2007.  Tickets are $100 apiece (plus $3 handling), are non-refundable but are transferable.

  4. Individuals may purchase a brick for the Space Park Recognition Wall from June 3, 2007 to June 23, 2007 will receive 25 entries per brick purchased.  Payment for the brick  ($500 per brick) must be received at the LVCC office (867 W Buckingham, Lower Level, Chicago, IL 60657) by midnight CST, June 23, 2007.

  5. The winner will be chosen by a random drawing the week of June 24, 2007 and notified shortly thereafter.  The winner must verify they meet all the terms and conditions listed above before the two tickets will be awarded.  All decisions by the LVCC Executive Board are final.   The winner is responsible for all taxes associated with the prize.  Estimated retail value of the two tickets are $550. 

  6. The winner agrees to allow LVCC to use their name and likeness to promote the The Police Ticket Give Away.
